Premier Volleyball Club - Accept/Decline Team Spot

Thank you for trying out for Premier Volleyball Academy. Please fill in the accept or decline information below that will generate to the staff immediately with your decision. If you are accepting a spot at Premier, please make your $100 deposit here in order to secure your spot on the roster that you have been offered. This $100 deposit is applied to the total club dues. If you would like to mail in the $100 deposit, it must arrive by your tryout deadline to Premier Volleyball Academy, 1630 Market Place Dr. Maumee, OH 43537. Players that do not make deposits by the deadline will be dropped from the roster. All players that accept a spot on a Premier Team should plan to attend the Team Meeting Day for your team grade/level. Every player and at least one parent are required to attend this meeting to fill out paperwork, order uniforms, and make team payments.
